			<content:encoded><![CDATA[<p>Democrats&#8217; strategy for merging health care reform bills began to unravel under growing attacks Wednesday, including unexpected criticism from Colorado&#8217;s two Democratic senators.</p>
<p>Final negotiations on merging the bills began in earnest Wednesday as congressional leaders spent more than eight hours behind closed doors at the White House using a process that bypasses a formal conference committee and cuts out Republican participation.</p>
<p>They dug into a series of sticky issues that separate the two versions of the bill passed by the House and the Senate — including abortion, access to health insurance by illegal immigrants, and how to pay for the sweeping legislation, which will insure an estimated 37 million Americans over the next decade.</p>
<p><span id="more-143"></span></p>
<p>A White House statement said they had made &#8220;significant progress in bridging the remaining gaps&#8221; between the bills.</p>
<p>Republicans have been howling about the process for nearly two weeks — House Minority Leader John Boehner, R-Ohio, called it &#8220;a breeding ground for more of the kickbacks&#8221; — but now congressional leaders are facing a growing mutiny within their own ranks.</p>
<p>Wednesday, Sen. Mark Udall, D-Colo., suggested the talks lacked transparency and called for televising final negotiations. Sen. Michael Bennet, D-Colo., also said final talks should be open to live cameras.</p>
<p>&#8220;I haven&#8217;t been in Washington long, but one thing I can tell you is this is one town that can use some fresh air, and some real transparency,&#8221; Bennet said. &#8220;The outcomes from the negotiations on the health care reform bill are far too important to be done behind closed doors.&#8221;</p>
<p>Those criticisms could create significant difficulties for efforts to finish the health care bill and move on to other legislation before the press of this year&#8217;s election cycle swamps Congress.</p>
<p>Media organizations and others have said televising final negotiations between the House and the Senate would be natural given the enormous potential impact of the legislation.</p>
<p>Instead, Democrats have created a process under which House and Senate leaders will hammer out key compromises, then send the retooled bill to both chambers.</p>
<p>Those negotiations are expected to go for days, if not weeks, and Democratic leaders have said a conference would only provide Republicans — only one of whom voted for the bills in either chamber — a new chance at obstruction.</p>
<p>&#8220;This non-conference conference was a bid to speed this up and bring it to an end. A conference committee seemed one more area where Republicans would attack the Democrats and liberals would get upset with leadership,&#8221; said Julian Zelizer, an expert on Congress at Princeton University.</p>
<p>&#8220;The problem is it looked bad. Republicans can use that to say health care is being rushed through and done behind closed door because Democrats want to hide something,&#8221; he said. &#8220;That&#8217;s why you&#8217;re seeing Democrats now getting nervous about the process as well.&#8221;</p>

			<content:encoded><![CDATA[<p>U.S. Rep. Steve King, R-Iowa, said Democrats are working to create a “dependency class” in America in an effort to expand their political base and stay in power.</p>
<p>“That’s part of the motive,” King said when discussing federal health-care reform efforts with reporters after a Tuesday taping of Iowa Public Television’s “Iowa Press.”</p>
<p>King, who represents Iowa’s 5th District, said he will do what he can to try stop a health-care bill from heading to President Barack Obama’s desk, and he urged others who opposed the bill to join him.</p>
<p><span id="more-137"></span></p>
<p>He said Democrats are moving toward nation health care, whether a public insurance option is included in the final bill or not.</p>
<p>“That’s the goal; that’s the endeavor,” King said. “They’ll regulate everything, and when they do that, we will lose the liberty we have today to buy health insurance policies.”</p>
<p>He predicted that if Congress passes health-care reform, Democrats will pay a price at the ballot box in 2010.</p>
<p>“I’ve never seen this kind of energy in America, this kind of uprising, especially from the heart of the heartland of America,” King said.</p>
<p>King said he is worried about the “mindset” drifting into America that doesn’t seem to understand the free-enterprise system.</p>
<p>“We’re descendants in this part of the country from people who came across America in covered wagons,” King said. “I mean, they came here to live free or die on the prairie. They didn’t ask for a government handout.”    </p>
<p>Norm Sterzenbach, executive director of the Iowa Democratic Party, called King’s comments hypocritical.</p>
<p>“Before he rails against Democrats for working to help seniors pay for prescriptions and help students afford college, he should consider giving up his government salary, as other members of Congress have,” Sterzenbach said.</p>
<p>King, a four-term congressman, said he plans to seek another term in 2010. Iowa is expected to lose one of its congressional seats, and King said he probably still will seek re-election in 2012 even if redistricting places him in the same district with another member of Congress.</p>
<p>As Iowa Republicans look to unseat Gov. Chet Culver next year, King said he did not know whether he would endorse one of the candidates in the GOP primary.</p>
<p>“I’d like to see them fight this out, because it tests their vigor, and it tests their ability, and it also shapes the policy for Republicans that will be matched up against the policy that’s been set by Gov. Culver,” King said.</p>
<p>The “Iowa Press”  featuring King is scheduled to air at 7:30 p.m. Friday and noon Sunday on Iowa Public Television.</p>

Malignant nodules are cancer. They are generally more serious and may sometimes be life threatening. Cancer cells can invade and damage nearby tissues and organs. Also, cancer cells can break away from a malignant nodule and enter the bloodstream or the lymphatic system. That is how cancer spreads from the original cancer (primary tumor) to form new tumors in other organs. The spread of cancer is called metastasis.
<p>Most often the first symptom of <strong>Thyroid Cancer</strong> is a nodule in the thyroid region of the neck. However, many adults have small nodules in their thyroids, but typically fewer than 5% of these nodules are found to be malignant. Sometimes the first sign is an enlarged lymph node. Later symptoms that can be present are pain in the anterior region of the neck and changes in voice. Thyroid Cancer is usually found in a euthyroid patient, but symptoms of hyperthyroidism or hypothyroidism may be associated with a large or metastatic well-differentiated tumor. Nodules are of particular concern when they are found in those under the age of 20. The presentation of benign nodules at this age is less likely, and thus the potential for malignancy is far greater.</p>
<p>The following are the major types of thyroid cancer -</p>
<p> Papillary and follicular thyroid cancers account for 80 to 90 percent of all thyroid cancers. Both types begin in the follicular cells of the thyroid. Most papillary and follicular thyroid cancers tend to grow slowly. If they are detected early, most can be treated successfully. Medullary <strong>Thyroid Cancer</strong> accounts for 5 to 10 percent of thyroid cancer cases. It arises in C cells, not follicular cells. Medullary thyroid cancer is easier to control if it is found and treated before it spreads to other parts of the body. Anaplastic <strong>Thyroid Cancer</strong> is the least common type of thyroid cancer (only 1 to 2 percent of cases). It arises in the follicular cells. The cancer cells are highly abnormal and difficult to recognize. This type of cancer is usually very hard to control because the cancer cells tend to grow and spread very quickly.
<p>If the nodule is benign, patients may receive thyroxin therapy to suppress thyroid-stimulating hormone and should be re-evaluated in 6 months. If the nodule is malignant or has indeterminate cytologic features, it may require surgery. Common <strong>Thyroid Cancer </strong>surgeries include thyroidectomy, lobectomy, and tracheostomy. Radioactive Iodine-131 is used in patients with papillary or follicular thyroid cancer for ablation of residual thyroid tissue after surgery and for the treatment of thyroid cancer. Patients with medullary, anaplastic, and most Hurtle cell cancers do not benefit from this therapy. 			<content:encoded><![CDATA[<p>Oleta Fitzgerald, director of the Children&#8217;s Defense Fund&#8217;s Southern Regional Office, says she is concerned over the welfare of Mississippi children if either of the two health-care reform packages considered by the U.S. House and Senate ever make it into law.</p>
<p>The House passed H.R. 3962 earlier this month, and Senate Democrats managed to beat back the threat of a Republican filibuster a few weeks ago, allowing the Senate to move forward with debate on the Patient Protection and Affordable Care Act, H.R. 3590. Both bills promise big reforms in the health-care and health-insurance industries. The Association for American Medical Colleges states that nearly 15 million people will be newly eligible for Medicaid and the Children&#8217;s Health Insurance Program under H.R. 3590, at an estimated cost of $374 billion over 10 years.</p>
<p><span id="more-120"></span></p>
<p>Fitzgerald says both bills contain huge holes regarding CHIP coverage for Mississippi children: &#8220;Right now, the fight over health-care reform in the House and Senate is all about abortion and the public option, but the children are getting lost in this discussion,&#8221; Fitzgerald said.</p>
<p>The issue, she said, centers on Mississippi&#8217;s unconventional requirement for CHIP eligibility.</p>
<p>Many states recently expanded their Medicaid program requirements to accept people who are a little further from the federal standard for poverty. Eleven states recently extended CHIP-eligible families&#8217; income levels up to 200 percent of the federal poverty level, or higher. ($20,800 for an individual or $35,200 for a family of three).</p>
<p>But instead of expanding Medicaid, Mississippi set up a new health insurance program that contracts with private insurance companies. The states that expanded Medicaid will continue to receive federal support for those programs under both the bills under discussion in the House and Senate. But in Mississippi, all children and their families over 150 percent of the federal poverty level ($16,245 a year for an individual and $27,465 a year for a family of three) would go into an insurance exchange created by the House and Senate bills. The Senate bill plans to put CHIP-eligible kids in an exchange by the year 2019, while the House bill has them transferred by 2013.</p>
<p>Insurance exchanges do not promise the reliability of a government health program, Fitzgerald warns.</p>
<p>&#8220;Going into the exchange could require co-pays and premiums, the children would get lumped in with adults, and it&#8217;s not clear what requirements the insurance companies would have for their benefit packages,&#8221; she said.</p>
<p>There is also the question of permanence. Exchanges like the ones proposed by the House and Senate bills have not always been long-lasting. Texas, Florida, North Carolina and California all attempted—and failed—to create enduring insurance exchanges, primarily because private insurers tampered with the market.</p>
<p>A July report issued by the California HealthCare Foundation tried to pinpoint some of the factors that killed the California insurance exchange, which closed its doors in 2006. According to the report, the California exchange became too expensive when the clients it served became too costly. An exchange requires a certain number of healthy individuals to complement the more sickly participants of the exchange&#8217;s customer base; otherwise the cost of participation becomes too high for all participants.</p>
<p>But insurance companies in California lured healthy customers with lower premiums and steered the more sickly individuals into the exchange, creating a disproportionately expensive customer base.</p>
<p>&#8220;People involved in operations of the California exchange agreed that when there is competition for the same customers within and outside the exchange, the exchange is in &#8216;extreme peril&#8217; of becoming a victim of adverse selection,&#8221; the report states. &#8220;If an exchange attracts a disproportionate share of higher risk individuals and groups as the California exchange did at various times, it cannot succeed.&#8221;</p>
<p>Fitzgerald said Mississippi&#8217;s eagerness to boot CHIP-eligible children from the program to keep down state costs is another factor complicating the new bills.</p>
<p>&#8220;Another problem is enrollment. We need enrollment in the exchanges to be simplified, because enrolling in state health programs have a history of being anything but simple in Mississippi,&#8221; Fitzgerald said, referencing a Medicaid policy championed by Republican Gov. Haley Barbour, which requires Medicaid recipients to meet Medicaid personnel &#8220;face-to-face&#8221; to be considered for program renewal.</p>
<p>CDF is working with its national office in trying to insert an amendment in the Senate bill though Democratic Sens. Robert Casey and Jay Rockefeller, which would keep all children up to 300 percent of the federal poverty level in the CHIP program until the new insurance exchange is thoroughly vetted.</p>

Jack Markell of Delaware said in an interview Wednesday, hours before getting elected as the chairman of the Democratic Governors Association. “And we’re doing our best to communicate them. We understand the need to get something done, and we’re supportive of getting something done. But we want to make sure it’s done in a way that state budgets are not negatively impacted.”</p>
<p><span id="more-116"></span></p>
<p>From the start, Republican governors have been more outspokenly critical about the health care legislation – in particular, the bill proposed by Harry Reid of Nevada, the Senate majority leader – which they said would saddle them with millions of dollars in additional Medicaid costs as insurance coverage is expanded. At their own meeting two weeks ago in Texas, Republican governors declared Democrats felt the same way as they did, but were less apt to say it out of loyalty to President Obama.</p>
<p>Asked about that, Mr. Markell responded: “Perhaps we’ve expressed some of our concerns less publicly. But I believe all governors are certainly concerned about what the potential impact is of some of these bills.”</p>
<p>Mr. Markell said that there was no division between governors and the administration on the need to get some sort of health care bill through; he said that he was reminded of the need in conversations with small businesses struggling with health care costs and constituents who have been unable to get health care coverage. He said his concern was some of the bills being considered would do that by shifting some of the costs to the state – but said he remained confident, after conversations with the White House, that would not be the case.</p>
<p>Whatever the outcome of the health care deliberations, Mr. Markell said he did not believe it would affect the electoral outcome for governors in 2010, a year in which 19 gubernatorial seats currently held by Democrats are on the ballot. The key issues, the governor said, were jobs and the economy.</p>
<p>And to that regard, Mr. Markell said that he was hopeful that the White House and Congress would dispose of the health care deliberations and move on to discussing some sort of jobs creation legislation.</p>
<p>“Right now I believe we need to be focused really significantly on the state level on jobs and on the economic climate overall,” he said. Asked if Mr. Markell thought Mr. Obama and Congress were spending too much time on health care at the expense of the economy, he responded: “Well I feel it would be terrific if they could finish health care and move on.”</p>

			<content:encoded><![CDATA[<p>West Virginia&#8217;s health care system could save over $1.1 billion by going digital and centralizing patient care, according to a first-of-its-kind report presented to lawmakers Monday.</p>
<p>Those savings would be seen not just by government agencies, but by private insurers and policyholders, who could benefit directly in the form of lower premiums.</p>
<p>The report should lend urgency to some initiatives that have already begun, like electronic medical records and prescriptions, according to the groups behind its creation.</p>
<p><span id="more-109"></span></p>
<p>&#8220;It really is remarkable to consider the savings available from options that are, basically, low-hanging fruit,&#8221; said Perry Bryant, executive director of West Virginians for Affordable Health Care.</p>
<p>Lawmakers with a joint interim committee that oversees health care policy responded Monday with cautious praise for the report.</p>
<p>&#8220;I think it&#8217;s very positive, and certainly provocative,&#8221; said House Health and Human Resources Chairman Don Perdue, D-Wayne. &#8220;It&#8217;s gratifying to see that once again, West Virginia is ahead of the game.&#8221;</p>
<p>The estimates in the report, prepared by CCRC Actuaries for the West Virginia Health Care Authority, used insurance claims data from more than 800,000 West Virginia residents, including people in public plans like Medicaid and private plans like Mountain State Blue Cross Blue Shield.</p>
<p>Both the volume and the range of information make the report&#8217;s estimates uniquely valuable, Bryant said.</p>
<p>&#8220;I don&#8217;t know of any other state where private insurers have voluntarily pooled their data,&#8221; he said.</p>
<p>The three pieces of &#8220;low-hanging fruit&#8221; in the report are electronic prescribing, digital medical records and the so-called &#8220;medical home&#8221; concept of patient care, which prizes close relationships between patients and doctors to provide a broad spectrum of care.</p>
<p>West Virginia has already made steps to adopt these strategies, but the report&#8217;s estimates are based on their statewide implementation.</p>
<p>In the case of electronic prescriptions, the report estimates an overall savings of $164 million in 2014, including nearly $51 million in savings to private insurers and $42 million in savings to policyholders.After that, the fruit doesn&#8217;t hang quite so low. The report estimates that a statewide rollout of medical homes would cost about $45 million up front and incur ongoing costs of about $368 million.</p>
<p>When subtracted from the estimated 2014 savings of $643 million, though, that still means an overall break of roughly $274 million, with the report estimating that savings growing to nearly $2 billion in 2019.</p>
<p>The most difficult of the three measures to implement is a statewide system of electronic medical records. West Virginia has already installed digital record keeping systems at seven state hospitals, but getting private physicians on board is not so easy.</p>
<p>Estimates suggest that about nine in 10 health care offices still keep everything in paper. As the new report says, up front costs for physicians run from $25,000 to $45,000 and have annual costs thereafter of between $2,000 and $9,000, steep amounts for small practices.</p>
<p>If electronic medical records are adopted statewide in the next four years, though, the report estimates a savings of more than $317 million, including $85 million for private insurers and $84 million for policyholders.</p>
<p>Lawmakers at Monday&#8217;s interim meeting questioned whether the federal health care legislation in Congress might complicate the West Virginia efforts cited in the report.</p>
<p>&#8220;I&#8217;m concerned about the blending of the two,&#8221; Senate Health and Human Resources Chairman Roman Prezioso, D-Marion, told Bryant. &#8220;I&#8217;m glad to hear you say we&#8217;re ahead of the curve. I didn&#8217;t anticipate that.&#8221;</p>
<p>Bryant urged legislators to press ahead with the state&#8217;s initiatives, particularly the medical homes, whether or if something emerges from Capitol Hill.</p>

<p>The agency that generally provides health care to the underserved has received $706,264 from the federal government to create a program to enroll thousands of children in either of two government insurance programs for low-income children.</p>
<p>Many metro-area children are eligible but aren&#8217;t enrolled because their families don&#8217;t know the programs exist or don&#8217;t know their kids could qualify, said Andrea Skolkin, chief executive officer of OneWorld. That means some of those children are going without health care or are getting far less than they could.</p>
<p><span id="more-91"></span></p>
<p>OneWorld&#8217;s goal is to enroll at least 6,000 children. The agency&#8217;s outreach effort has just begun.OneWorld will place staff members in day care centers, schools, after-school programs, churches, food pantries, organizations and other places.</p>
<p>“We want to be where people are versus making people come to us,” Skolkin said.</p>
<p>They will contact families at those sites and determine whether they have children who qualify but aren&#8217;t enrolled in Medicaid or the state&#8217;s Children&#8217;s Health Insurance Program.</p>
<p>The staff members will have laptops to take down information and scanners to scan in citizenship documents and proof of Nebraska residency. Children must be citizens to receive the health care benefits.</p>
<p>The agency also will take referrals. For information, call 502-8888.</p>
<p>OneWorld, based in the Livestock Exchange Building, 4920 S. 30th St., has hired a director and will employ five full-time staffers for the program. OneWorld also has a clinic in Plattsmouth.</p>
<p>President Barack Obama this year allocated $40 million to agencies in 42 states and Washington, D.C., for programs to conduct enrollment efforts over the next two years.</p>
<p>Through a competitive process, OneWorld was one of 69 entities to receive money. Iowa doesn&#8217;t have a program among the 69. An additional $40 million will be distributed in 2012.</p>
<p>Enrollment among children in Medicaid and the Children&#8217;s Health Insurance Program has gradually risen in Iowa and Nebraska. The economy has worsened and awareness of the programs has broadened, spokesmen in Iowa and Nebraska say.</p>
<p>A child qualifies for Medicaid if his family&#8217;s annual income is at or somewhat above the federal poverty level, which is $18,310 for a family of three.</p>
<p>Qualifying for CHIP isn&#8217;t as stringent. In Iowa, the state raised the CHIP ceiling this year to 300 percent of the federal poverty level, or $54,930 for a family of three. Nebraska raised its income ceiling for CHIP from 185 percent this year to 200 percent, or $36,620 for a family of three.</p>
